How Do I.... Register With The Practice?

To register, please ask our receptionists for a questionnaire. You will be asked some routine questions about where you live and who is your preferred GP.

Carers Register

Please tell our receptionists if you care for someone who has an illness, or you have a carer looking after you.

How Do I.... Change My Details?

It is most important that you inform us as soon as possible if any of your personal circumstances change; for example, you change your name upon marriage, have a new telephone number or you move to a new address. Please do note that if you move to an address outside tha practice area, it will be nessecary for you to register with another practice in your new locale.

How Do I.... Request Repeat Prescriptions?

These can only be obtained when authorised by your doctor. Please give at least 24 hours' notice. All patients having repeat prescriptions will be required to see their doctor at regular intervals.
They can only be obtained in the following ways:

By post: a stamped, addressed envelope must be included if you want the prescription returned to you by post.

Personally: by placing your request in the box provided in the waiting area

They cannot be requested by telephone.
